As a critical document, you should report a missing passport as soon as possible.If you misplace your passport, you can apply for a new one.Even if you lost your passport abroad, this is still true.
Step 1: You can apply in person.
You can apply for a new passport in person by calling the nearest Passport Agency or Passport Application Acceptance Facility.You need to schedule an appointment within the next five business days at a Passport Agency or Center if you are traveling abroad within two weeks.If you are not traveling abroad within two weeks, schedule an appointment with any authorized Passport agency.Here you can find a list of Passport Application Acceptance Facilities.
Step 2: Form DS-11 needs to be completed and submitted.
The application is for a U.S. passport.When you lose your passport, you must apply for a new one using the standard form.If you fill it out on your computer, you can either print it or hand it in.You must bring the completed form with you to your appointment.Form DS-11 can be found here.
Step 3: Form DS-64 needs to be completed and submitted.
The statement is about a lost or stolen passport.You can either complete the form by hand or on the computer if you bring the completed form to your appointment.The forms must be printed out before your appointment.Form DS-64 can be found here: https://eforms.state.gov/forms/ds64.
Step 4: To make your appointment, bring all the necessary documents.
You need to bring your identification with you to your appointment.Proof of citizenship, proof of identity, and a recent color photograph are some of the documents you need.If you want to know the current fee for a standard adult passport book, you should visit the travel.state.gov.
Step 5: Call the nearest U.S. embassy.
You must schedule an appointment with the nearest U.S. embassy or consulate if you want to replace your passport before you can return to the United States.You can find the nearest embassy by clicking here: http://travel.state.gov/content/passports/english/country.htmlYou can find the nearest location by looking under the "Embassies and Consulates" section on the country's website.
Step 6: Inform the Consular Section if you have any interesting information.
The section of the embassy is equipped to handle passport troubles.You should specify if your passport is lost or stolen.If you need to leave the foreign country within a short period of time, let the staff know.
Step 7: Take a picture.
A new photograph is usually needed for your replacement passport.The staff at the embassy should be able to give you a location where you can get a photograph.You need a photo before you can go to the Consular Section in person.The picture must have a full view of your face.It should measure 5 cm in size.
Step 8: All the necessary documents should be brought.
You will need to show proof of your U.S. citizenship, your travel itinerary, and a police report in order to get a visa.Forms DS-11 and DS-64 need to be completed as well.A driver's license or expired passport can be used as identification, and a missing passport or birth certificate can serve as proof of citizenship.Show airline or train tickets to prove your travel itinerary.A police report can be used to confirm your circumstances.If you spend time filing and getting a police report, your travel plans won't be affected.Forms DS-11 and DS-64 can be completed at the time of your application if you can't do it before.It is possible that you will need to pay a fee for your replacement passport.The fee for a standard adult passport book will be $135 as of 2014, if you check on the current fee.
Step 9: What do you need?
A standard replacement passport can be obtained if you don't need a passport immediately.You will need to request a limited-validity emergency passport if you have urgent travel plans.A standard replacement is valid for a decade for adults and five years for children.You will only be able to return to the U.S. if an emergency replacement is produced more quickly.If you want to get a standard replacement, you will need to turn in your emergency passport.
Step 10: Unless you have an emergency, don't call on a business day.
Most of the U.S. embassies won't issue passports on weekends or holidays.If you have a life-or-death emergency or have been the victim of a serious crime, you can still contact the embassy or consulate after hours.Duty officers only respond to the most serious emergencies.If you need to return to the United States for medical treatment, the officer can help you get a replacement passport.
Step 11: Know how to help someone abroad.
If a loved one tells you that their passport is missing, you can help them by contacting the Office of Overseas Citizens Services at the U.S. Department of State in Washington, D.C.If you want to get in touch with your loved one, you should call the office.Your loved one will need to complete the rest of the process in person and on his or her own, but the consulate staff will be there to help.
Step 12: You can report the lost passport by phone.
If you don't want to replace your passport, you can call the US State Department toll free.
Step 13: Form DS-64 needs to be completed and filed.
If you want to keep a paper trail of your report, you should complete Form DS-64, Statement Regarding Lost or Stolen Passport.If you only want to report the lost passport, you don't need to file Form DS-11, Application for a U.S. Passport.The completed Form DS-64 can be mailed to the US Department of State.
Step 14: Report the non-receipt of a passport.
If you haven't received your new U.S. passport, you should contact the National Passport Information Center.Wait until you get a notification that your passport has been mailed to you.You can seek help if you don't receive it within ten business days.You will need to pay the fees again if you don't report this non-receipt within 90 days after your passport was issued.Send an e-mail to: NPIC@state.gov if you want to contact the National Passport Information Center.
Step 15: You can mail lost passports.
